IMPORTANT DATES:
·         Last date of submission of application form - 15th of December 2016.
·         Date of screening for S.No 1 shall be notified separately in leading Daily news paper(s) of state.
The applications should be addressed to Chief Medical officer (Vice Chairman) DHS/NHM, Rajouri on the prescribed format. Interested candidates should apply on the format given below along with attested copies of supporting documents/Certificates which must reach in the Office of the Chief Medical Officer Rajouri by post or by hand against proper receipt latest by - 15th of December 2016.


CONTRACTUAL TERMS:
01. Contract will be offered by District Health Society, Rajouri.
02. Remuneration mentioned is all inclusive.
03. The appointees shall not be entitled to any preferential claim for regular appointment under normal process of selection
04. The appointee shall not be entitled to any other allowances or monetary benefits whatsoever.
05. The hiring is purely on contractual basis initially upto 31.03.2017 which may be renewed for 1year more, based on performance and extension of scheme/programme. In case of
unsatisfactory performance the district health society shall have the right to terminate the services of appointees.
06 The appointee shall have to work at the place and against the post to which they have been appointed and shall not be eligible for transfer from his/her initial place of posting.
07 The candidate shall have to sign a contract with the District Health Society as per the guidelines under National Health Mission.
08. Contract is liable to be terminated prematurely without any prior notice in event of any misconduct or inefficiency on part of the candidate.
09. Application can be rejected by the screening committee if found not falling in the prescribed Criteria.
10. Interested persons may apply by sending their applications in the prescribed format given below and the duly filled in application form must reach in the office of the Chief Medical
Officer, Rajouri on or before by - 15th of December 2016.
11. Application form of candidates from outside the district Rajouri will not be entertained.
12. Locals shall be given preference and "Local Criteria" as defined in the frame work of Implementation of NHM implies local Residence with in Village/Tehsil/Block and District as per
availability.
13. The selected candidate shall have to sign a contract with DHS, Rajouri.
14. Posts can be increased or decreased at any point of recruitment process.
15. No call letters will be issued in this regard, Interview Postponement/Pre-ponement Notification, Selection List, etc will be published in daily newspaper(s).
16. Forms not accompanied with all the documents mentioned, will not be accepted.
17. No TA/DA will be admissible to the candidates coming for interview.
18. The hiring shall also be governed by other Terms & Conditions which are not specifically mentioned herein, but are made applicable in case of such appointments at any time by
DHS.
