Terms & Conditions:

1. The candidate must be domicile of UT of Jammu and Kashmir.

2. The consolidated Honorarium per month is inclusive of all allowances and benefits.

3. The selection of candidate for each post shall be as per the criteria conveyed by State Health Society, NHM vide its letter No.SHS/NHM/J&K/13543-53 dated 14.10.2020 and SHS/NHM/J&K/13654-65,dated16.10.2020.

4. The selected candidate shall have to submit an affidavit to the effect that he/she shall not claim for any regular appointment/ extension in the department.

5. The posts are on hiring basis and temporary: do not confer any right for the selected candidates to the permanent absorption in the department.

6. The engagement shall be terminated on account of non performance without any notice & keeping in view the requirement of staff for COVID-19.

5. After successful completion of the application you will receive an email and you will be able to get the print out of your filled form.

6. Application form once submitted successfully will not be editable. Candidate will be solely responsible for all the information submitted by him/her. In case of any information is found incorrect/ deficient/incoherent at any stage, the application form shall be summarily rejected and such candidate will be debarred from all the future engagements.

Online applications are invited from eligible candidates (belonging to Kashmir Division) for engagement as ANM on Academic Arrangement Basis under SRO. 24 of 2019 dated. 10-01-2019 and under National Health Mission Programme for L.D Hospital, Sgr. on the terms and conditions listed below:

Terms and conditions:

SCHEME-A (31 POSTS ON ACADEMIC ARRANGEMENT BASIS)
1	Nature of engagement	Contractual as defined in SRO-24 of 2019 dated: 10.01.2019.
2	Period	For a period of one year or till the posts are filled up by the competent authority whichever is earlier.
3	Salary	The engagee shall be paid salary at the minimum of pay scale in accordance with SRO 24 of 2019, dated:-10-01-2019.
4	Engagement	The engagement against the advertised posts shall and shall always be on Academic Arrangement Basis only without conferment of any preferential right on the engagees for regular appointment against these posts which shall be made strictly in accordance with the relevant recruitment rules.
5	Agreement	The selected candidates shall have to enter into an agreement with this Institution to the effect that they will not leave the job before the completion of one year's contract. Abandon- ing/terminating of the agreement, as the case may be, will require one month's prior notice on either side failing which the salary will not be paid for one month to the incumbent which shall always remain in deposit with the department and shall be released on completion of sanctioned term.
6	Age	The age of the applicant as on 01.01.2020 must not be: Below 18 years; and Above: 40 years in case of Open Merit 63 years in case of retired Servicemen 43 years in case of reserved category candidates.
SCHEME -B (30 POSITIONS UNDER NHM FOR L.D HOSPITAL. SGR.
1.	Nature of engagement	Contractual
1	Period	Initial contract for hiring shall be upto 31.03.2021, which shall be renewable for a further period of one year or till continuation of the scheme whichever is earlier, subject to satisfac­tory performance of thecandidates/approval of the competent authority. The contractual hiring shall not confer any right upon the selected candidates to claim regu- larization/permanent absorption in the institution.
2	Honorarium	Rs. 12000/- consolidated. P.M The consolidated honorarium per month is inclusive of all allowances and monetary benefits.
3	Agreement	The selected candidates shall have to enter into an agreement with this Institution to the effect that they will not leave the job before the completion of the financial year or till continuation of scheme. Abandoning/terminating of the agreement, as the case may be, will require one month prior notice on either side failing which the salary will not be paid for one month to the incumbent which shall always remain in deposit with the department and shall be released on completion of sanctioned term.
4	Age	1. The age of the applicant as on 01.01.2020 must not be: i) Below 18 years; and Ii) Above: 40 years in case of Open Merit 43 years in case of reserved category candidates.

Note:

1. The candidates must be in possession of all required documents and no document/ certificate issued beyond the cut off date shall be accepted/entertained.

2. The selection shall be made purely on the basis of merit obtained by the candidate in the written test (to be held subsequently) and the candidate with higher merit shall be considered for Academic Arrangement Basis and those with comparatively lower merit shall be considered under National Health Mission Programme.

2. A link for filling up of online application form shall be available on the official website of GMC, Sgr.

3. Candidates are required to apply online through the official website of GMC, Sgr. only.

4. The requisite original documents including domicile certificate issued by the competent authority on the format prescribed for the purpose must be produced at the time of document verification, or as and when sought by GMC, Sgr.

5. The candidates are required to upload the images of their recent photograph and specimen signature which should be uploaded in (Jpeg) format only.

6.Candidates have to deposit fee of Rs. 300/- through online mode only i.e through Net Banking/Debit Card/Credit Card.

7. Applications of those candidates will not be considered who do not pay the fee in online mode.

8. Candidates must take a print out of the application form and fee receipt after successful filling up of the application form.

9. Candidates can not edit their application form after submission.

10. Candidates shall ensure that their E-mail ID and Phone Nos. as specified in the online application from are active during the currency of selection process.

11. Candidates should not submit a print out of the application/fee payment receipt to GMC at this stage. However, the candidates shall submit the downloaded printout of the application forms alongwith the fee payment receipts and attested copies of requisite certificates as and when GMC may call for submission of such documents.
